Primary Care Plus offers a friendly work environment, competitive salaries and an excellent benefits package. Primary Care Plus is affiliated with Peoples Health, and some of the benefits are administered through them. Medical coverage is offered through the PHN Employee Plan.

Medical Coverage Overview
Primary Care Plus, through its affiliate Peoples Health, offers broad-scale medical coverage to ensure that you and your family have access to rich healthcare benefits. The plan allows you to coordinate your healthcare within the network of Peoples Health providers as well as out of the network. You also may elect to provide coverage for your spouse or domestic partner and your children. The plan covers:
- Physician and specialist office visits
- Emergency room care
- Urgent care
- Diagnostic tests and X-rays
- Prescription drugs
- Maternity needs
- Pediatric needs
- Routine physical exams
Medical coverage through the plan also provides you and your covered dependents with a free membership to a Peoples Health participating fitness center.

VSP offers a convenient and affordable way to ensure the health of your eyes. This plan provides coverage for routine eye examinations, frames, lenses, contacts and laser vision surgery. You also may elect to provide coverage for your spouse or domestic partner and dependent children.
From routine cleanings to extensive procedures, MetLife provides comprehensive dental benefits. MetLife has an extensive network of dental providers from which employees can choose to receive services. Employees may elect to provide coverage for their spouse or domestic partner and dependent children.
Employee Assistance Program
An employee assistance program is a counseling service to help improve the personal and professional life for you and your eligible dependents who may be experiencing personal or work-related problems. These services are provided at no cost to the employee. This program is provided through Hildalgo Health Associates.
As an employee, you are automatically enrolled in basic life insurance equal to one times your annual eligible earnings, rounded to the next highest $1,000, up to a maximum of $200,000. This rate includes accidental death and dismemberment coverage. This life insurance is provided at no cost to you. This insurance is offered through Lincoln Financial Group.
Life Insurance Supplemental
As an employee, you may elect supplemental term life insurance for yourself in $10,000 increments up to $120,000. You can also secure life insurance coverage for your spouse in multiples of $5,000 up to $60,000 and a minimum of $10,000 for each dependent child. This insurance is offered through Lincoln Financial Group.
Short-Term Disability Insurance
Short-term disability insurance is a supplemental insurance policy that provides up to 11 weeks of income replacement in the event that you are unable to work due to a disability. It is equivalent to 60 percent of the first $1,250 of your weekly insured pre-disability earnings, reduced by deductible income, with a plan maximum weekly benefit up to $750 per week. This insurance is provided through Lincoln Financial Group.
Long-Term Disability Insurance
All full-time and part-time employees are automatically enrolled in the long-term disability plan. The long-term disability plan is provided at no cost to you. For long-term disability, there is a 90-day waiting period before benefits are paid. If eligible, you will be paid 66 and 2/3 percent of the first $9,000 of your monthly pre-disability earnings, reduced by deductible income (e.g., work earnings, workers’ compensation, state disability, etc.), up to $6,000 per month. This insurance is provided through Lincoln Financial Group.
Health and Dependent Care Flexible Spending Accounts
Flexible spending accounts provide the opportunity to contribute money on a pre-tax basis that can be used to pay for health and dependent care expenses. Flexible spending accounts are administered by Gilsbar.
Accident and Critical Illness Insurance Products
Allstate insurance products provide the financial assistance to help you get back on your feet if you are faced with expensive medical treatments or economic hardship due to a loss of income. This coverage provides a direct, immediate cash benefit to be used as you see fit at a time when you need it the most. These products can be purchased for you and for your dependents.
With the 401(k) plan offered through Merrill Lynch, we work with you to plan for a financially stable retirement. It’s never too early to start planning, and there are certain incentives that come with saving. When investing funds in a 401(k), an employee’s contribution is deducted from their paycheck before taxes are withheld, meaning that they save money on taxes because their taxable income is lower.
Once an employee has been with Primary Care Plus for one year, the company will match their contribution dollar for dollar up to 5 percent of their biweekly salary.
Employees can enroll in the 401(k) plan beginning on the first of the month following their first 30 days of employment.
529 College Savings Plan
A 529 college savings plan is designed to encourage saving for future college costs. Employees pay no federal income taxes on their earnings while the account is invested and no federal income taxes when they withdraw money to pay for qualified higher education expenses such as tuition, room and board, mandatory fees, books, and computers. Investing in a 529 college savings plan is a great way to begin saving for a child’s future. Employees can also invest money into this plan to pay future education expenses for themselves, grandchildren or other family members. This savings plan is offered through the START Saving Program. There is a 30-day waiting period to begin contributing to the 529 college savings plan.
