Primary Care Plus offers broad-scale medical coverage to ensure that you and your family have access to rich healthcare benefits. Employees may coordinate their healthcare within the network of PHN Employee Plan providers as well as with out-of-network providers. Employees may elect to cover their spouses or dependent children.

For 2018, Lincoln Financial Group provides coverage for routine eye examinations, eyeglasses frames and lenses, contacts, and laser vision surgery. Employees may also elect to cover their spouse or dependent children. VSP provides this coverage in 2017.
For 2018, the Standard provides dental coverage, from routine cleanings to extensive procedures. Employees may elect to provide coverage for their spouse or dependent children. MetLife provides this coverage in 2017.
Employee Assistance Program
An employee assistance program is a counseling service to help improve the personal and professional life of employees and their eligible dependents who may be experiencing personal or work-related problems. These services are provided at no cost.
Employees are automatically enrolled in basic life insurance with coverage equal to one times their annual earnings rounded to the next highest $1,000, up to a maximum of $200,000. This includes accidental death and dismemberment coverage. The life insurance is provided through Lincoln Financial Group at no cost to employees.
Life Insurance Supplemental
Employees may elect supplemental term life insurance for themselves in $10,000 increments up to $120,000. They can also elect coverage for their spouse in multiples of $5,000, up to $60,000, and a minimum of $10,000 for each dependent child. This insurance is provided through Lincoln Financial Group.
Long-Term and Short-Term Disability Insurance
All full-time and part-time employees are automatically enrolled in long-term disabilityinsurance, which is provided at no cost to employees. Eligible employees will be paid 66 2/3 percent of the first $9,000 of monthly pre-disability earnings, reduced by deductible income (e.g., work earnings, workers’ compensation, state disability, etc.), up to $6,000 per month, in the event the employee is unable to work due to a long-term disability. This insurance is provided through Lincoln Financial Group.
Short-term disability is a supplemental insurance policy that provides up to 11 weeks of income replacement in the event an employee is unable to work due to a disability. It is equivalent to 60 percent of the first $1,250 of their weekly insured pre-disability earnings, reduced by deductible income, with a plan maximum benefit up to $750 per week. This insurance is provided through Lincoln Financial Group.
Medical and Dependent Care Flexible Spending Accounts
Flexible spending accounts provide the opportunity to contribute money on a pre-tax basis that can be used to pay for medical or dependent care expenses. Flexible spending accounts are administered by Gilsbar.
Accident Insurance, Critical Illness Insurance and Cancer Insurance Products
Allstate policies for accident insurance, critical illness insurance and cancer insurance provide the financial assistance to help employees get back on their feet if faced with expensive medical treatments or economic hardship due to a loss of income. Employees may elect to cover their spouses or dependent children through any policy.
A 401(k) plan through Merrill Lynch helps employees plan for a financially stable retirement.
The company will match employee contributions dollar for dollar, up to 5 percent of their bi-weekly salary, for employees who have been with the company for one year.

529 College Savings Plan
A 529 college savings plan is designed to encourage saving for future college costs. The plan offered is the START (Student Tuition Assistance and Revenue Trust) Saving program.
